St. Rose Family Medicine provides general medical care for people of all ages. Through every cycle of life, we are here for you. Our dedicated team of healthcare professionals will take the time to listen to you and get to know your family’s unique needs. We strive to develop a personal and long-term relationship with everyone we serve.

Your family practitioners can provide:

Women’s Care – including routine Pap, pelvic and breast exams
Newborn and Infant Care
General Adult Care
Annual physical exams
School, employment and DOT physicals
Well-child checkups with immunizations/KAN Be Healthy exams
Disease prevention
Ongoing treatment of chronic problems


Jose Renteria, APRN

Jose completed his Master of Science in Nursing at Wichita State University with an emphasis in Acute Care. He is board certified with the American Association of Critical-Care Nurses (AACN), and is licensed through the Kansas State Board of Nursing. Prior to becoming an Advanced Practice provider, Jose has been a licensed RN for 10 years.

Kristin L. Ohnmacht, PA-C

Kristin earned her Masters of Medical Science in physician assistant education at Saint Louis University.  She attended Kansas State University for her undergraduate education where she earned a Bachelors of Science in Biology degree.  She is certified by the National Commission on Certification of Physician Assistants.

She is licensed and board certified by the Kansas State Board of Healing Arts.  Prior to joining St. Rose Family Medicine, Kristin’s experience included 5 years as a physician assistant specializing in the areas of Family Medicine, Emergency Medicine, Urgent Care and Orthopedics.  Kristin is also a DOT Certified Medical Examiner.

Haley Gleason, APRN

Haley earned her Master of Science in Nursing at the University of Cincinnati. She completed her undergraduate education at Fort Hays State University earning a Bachelor of Science in Nursing. She is board certified through the American Nurses Credentialing Center(ANCC) and is licensed through the Kansas State Board of Nursing. Prior to becoming an Advanced Practice Nurse Practitioner, Haley had seven years of experience as a Registered Nurse.
